This form has been designed to make your life a little easier for you. This form can either be printed and filled out or you may type it out via Adobe Reader and then print or email your filled-out form. Please bring your form to the St. Stephen Catholic Church Office Monday – Thursday from 9 AM – 3 PM and Friday from 9 AM – 1 PM. along with your cash or check donation. Mass requests can still be called in or sent via email to lilia@saintstephencc.org.

Por favor, traiga su formulario a la Oficina de la Iglesia Católica de San Esteban de Lunes – Jueves de 9 AM – 3 PM y Viernes de 9 AM – 1 PM junto con su donación en efectivo o cheque.
